- The Smile Plan is not dental insurance. This is an in-office membership plan that has been designed to provide a way for our patients to obtain the dental care they need at the most affordable rates.2 FREE preventative cleanings per plan year
- The Smile Plan is not only a discount plan, it is also a membership plan that covers 12 months. You will need to renew your membership every 12 months from the date when you started.
- The Smile Plan is standalone coverage and is not to be combined with your commercial insurance benefits or used in a workers’ compensation treatment plan.
- The Smile Plan is good at our location only. Should you be referred to a specialist, the specialist will not offer or honor this discount.
- The Smile Plan will not be refunded regardless of whether Smile Plan benefits are utilized during the participation period.
- This plan is non-transferable.
- Family members cannot be substituted for another family member.
