The Smile Plan is not dental insurance. This is an in-office membership plan that has been designed to provide a way for our patients to obtain the dental care they need at the most affordable rates.
The Smile Plan is not only a discount plan, it is also a membership plan that covers 12 months. You will need to renew your membership every 12 months from the date when you started.
The Smile Plan is standalone coverage and is not to be combined with your commercial insurance benefits or used in a workers’ compensation treatment plan.
The Smile Plan is good at our location only. Should you be referred to a specialist, the specialist will not offer or honor this discount.
The Smile Plan will not be refunded regardless of whether Smile Plan benefits are utilized during the participation period.
This plan is non-transferable.
Family members cannot be substituted for another family member.
